I also understand that any activity deemed criminal will be a cause for suspension or termination from this organization.

I understand that Southern Africa's Chaplaincy International (SACI) is a conservative Christian based fellowship and teaches the doctrines of orthodox Christianity without compromise.

I understand that Southern Africa's Chaplaincy International (SACI), Inc. maintains the right to suspend or terminate ministerial licensing for any reason that violates the Southern Africa's Chaplaincy International (SACI), Inc. Code of Ethics, Statement of Faith and organizational by-laws.

I affirm that I have read the Qualifications for License and Ordination, that this application contains no willful misrepresentations or falsifications, and that this information is true and complete to the best of my knowledge. All chaplains upon signing this form acknowledge and accept full responsibility for their actions in the performance of their duties, and release Chaplains International, Inc. from all harm and liability.
